Payment Policy

Madison Dental Associates is always accepting new patients and will gladly file your dental insurance claims for you. We are “In Network” providers for many dental insurance plans. Please call our office if you have any questions regarding our acceptance of insurance. Patient portions are due at the time that services are renderred. We will gladly file a pre-approval to insurance for estimated patient portions if requested.


We must have 24 hours notice to cancel an appointment. Without 24 hours notice, we do reserve the right to charge a “no show” fee of $75 per appointment scheduled that day. We have a voicemail system on our phones, and we do advise calling even if you know it is after hours to leave a message.

After two “no show” appointments (appointment was not cancelled or cancelled in less than 24 hours from the time of the appointment), we will no longer provider our services; therefore, you will need to seek treatment at another office.

Late Policy

We strive to be on time with every patient; therefore, if our patients are not on time it compromises the treatment that can be rendered to others. Due to the way we schedule appointments, if you are 7 minutes or more late, we have to reschedule the appointment. If we have to reschedule, there is a $75 charge for missing the appointment. It will also be considered a “no show” appointment. After two “no show” appointments, we will no longer provide services; therefore, you will need to seek treatment at another office.
